Eligibility
All patients must meet the following guidelines in order to receive medical and/or dental services. Each patient is screened for eligibility at the clinic during the first appointment and re-screened every six months thereafter. During the first appointment if proper documentation is not brought in you will not be seen. Eligible patients are those with no health insurance of any kind with a financial household income of 200% or less of poverty federal guidelines.
The scope of care provided by the Clinic is limited due to the volunteer nature of our clinic. Our staff will select patients for registration based on our ability to professionally meet their overall healthcare needs. We cannot guarantee any patient will be admitted for care.Income Guidelines
In order to qualify economically, you must be able to prove that as a household that you make 200% or less of federal poverty guidelines. This means:

Financial Documentation
You must bring the following on the first visit or you will not be seen:
- Current picture identification
- Social security number
- Verification of household income (pay stubs, W-2 form, etc. from all working members of the house in which you live)
- Summary of monthly expenses (rent, food, utilities, etc.)
- It is also helpful to bring any medical or prescription history you may have.
If you do not speak English you need to bring a translator with you to your appointment
The following is an overview of those who will typically qualify:
- Those working without insurance coverage
- Those with seasonal or low paying jobs
- Those who work part time
- Those who are unemployed
- Those attempting to become self-sufficient while leaving public assistance
- Those who are homeless or residing in shelters, although we typically refer these patients to Terry Reilly Health Clinic Boise because they have a grant to serve this population.
Those who do not qualify:
- If you have Government Benefits: Medicaid, Medicare or VA coverage
- If you have medical insurance - private or group policy
- If you suffer from work-related injuries involving Worker's Compensation
- If you are uninsured but receive an adequate annual income to cover medical expenses
